Reducing Poverty Initiative: Return-to-Work Support
A pilot initiative running until March 12, 2028
Do you have patients who are temporarily off work due to injury or illness? If so, the Reducing Poverty Initiative may be able to help!
What we do:
• Provide a Certified Disability Management Professional (CDMP) who works with your patient to
create a custom return-to-work plan and provides ongoing assistance
• Help your patients transition back to work with access to fully funded treatment where
appropriate
• Offer supplemental health services and workplace accommodations tailored to individual
needs
Patients can apply by phone or online, and healthcare providers can refer through Pathways.
Ideal for patients accessing Medical EI, but being on Medical EI is not a requirement.

• Vancouver Island Communities – Comox Valley, Central Island, Cowichan Valley, Nanaimo,
Salt Spring Island
• Northern Interior Communities – Northern Interior Rural, Prince George
• Southern Interior Communities – Central Okanagan, South Okanagan Similkameen, Shuswap
North Okanagan
• Lower Mainland Communities – Abbotsford, Burnaby, Delta, Fraser, Northwest, Langley, North
Shore, Richmond, Ridge Meadows, Sea to Sky, Sunshine Coast, Surrey/North Delta, White
